The order of the entrance to the dolphin therapy course
The patients are accepted to the dolphin therapy according to the pre-registration, after receiving and studying of your inquiry and medical documents by the doctors.
Needed medical documents (in copies, sent please by registered post letter):
- Extract from the child's case history
- Excerpt about the child from the medical institution domiciliary
- Excerpt about the child from special medical centers
- The data of the latter EEG-test (done not more than 6 months ago)
- Photo of the child
- Other medical documents
